Consent for Treatment

Voluntary Consent to Treatment

I voluntarily consent to receive mental health and/or substance abuse treatment services from Vantage Point Behavioral Health LLC ("VPBH"). I understand that treatment is voluntary and that I may withdraw my consent and discontinue treatment at any time.

Nature and Purpose of Treatment

I understand that my treatment may include the following services:

• Individual therapy/counseling sessions
• Group therapy sessions
• Medication management and psychiatric evaluation
• Case management services
• Psychiatric rehabilitation services
• Substance abuse treatment and counseling
• Crisis intervention services
• Other therapeutic interventions as clinically indicated

Benefits and Risks of Treatment

Potential Benefits:

• Improved mental health and emotional well-being
• Better coping skills and stress management
• Enhanced relationships and social functioning
• Reduction in symptoms of mental illness or substance abuse
• Increased self-awareness and personal growth

Potential Risks:

• Emotional discomfort when discussing difficult issues
• Temporary increase in distress as treatment progresses
• No guarantee of specific outcomes or improvement
• Potential side effects from medications (if prescribed)
• Treatment may not be effective for all individuals

My Rights and Responsibilities

I have the right to:

• Be treated with respect and dignity
• Receive services in a safe and therapeutic environment
• Participate in the development of my treatment plan
• Ask questions about my treatment and have them answered
• Refuse specific treatments or services
• Request a second opinion
• File a grievance or complaint about my care
• Receive culturally competent and individualized care

I agree to:

• Actively participate in my treatment plan
• Attend scheduled appointments or notify staff of cancellations
• Follow treatment recommendations to the best of my ability
• Communicate openly and honestly with my treatment team
• Treat staff and other patients with respect
• Comply with program rules and policies
• Provide accurate information about my medical history and current concerns

Confidentiality

I understand that all information disclosed during treatment is confidential and will be protected in accordance with federal and state privacy laws (HIPAA, 42 CFR Part 2). Information may only be released with my written consent, except in the following circumstances:

• When there is reasonable suspicion of child or elder abuse
• When there is imminent danger of harm to self or others
• When a valid court order requires disclosure
• For purposes of coordination of care within VPBH treatment team
• For billing and payment purposes
